During the registration process information obtained from you on the form is used to process your interest in potentially becoming an Ambassador or Advocate, such as contact information (name, address, e-mail address), age, date of birth, gender, ethnicity and medical condition. We will refer to your personally identifiable information to assist us in evaluating your potential role as a Patient Ambassador or an Advocate and may contact you to discuss further.

You must be 18 years of age or older to submit registration or survey information. If you are under the age of 18 and wish to register, then your parent or legal guardian must register on your behalf.
